Avaliação 3D dos arcos dentários de crianças com fissura labiopalatina: A influência da técnica cirúrgica

O propósito deste estudo foi avaliar o efeito das cirurgias plásticas, queiloplastia e palatoplastia, realizadas em tempo único e em dois tempos, nos maxilares de crianças com fissura labiopalatina. Foram selecionados 2 grupos: Grupo 1 (G1) - 26 crianças submetidas a queiloplastia com a técnica de Millard (TM) aos 3 meses, e palatoplastia pela técnica de Von Langenbeck, em etapa única, aos 12 meses; e Grupo 2 (G2) - 21 crianças submetidas a queiloplastia com TM e palatoplastia anterior pela técnica de Hans Pichler, aos 3 meses; e palatoplastia posterior pela técnica de Sommerlad, aos 12 meses. A avaliação foi realizada em imagens digitais dos arcos dentários, nos tempos: Tempo 1 (T1), antes da cirurgia, e Tempo 2 (T2), a partir dos 5 anos. Medidas lineares (I-C', I-C, C'-C, I-T', I-T, T'-T, C'-T', C-T), angulares (IC'T', ICT, C'T'T, CTT'), área do palato e análise oclusal foram obtidas pelo software de estereofotogrametria. Os testes de ICC, Shapiro-Wilk, ANOVA, Tukey, T independente e Mann-Whitney foram aplicados (α=5%). Em G1, a comparação intragrupo mostrou médias I-C', ICT, C'T'T, CTT' menores em T1 (p=0,002/p=0,001), enquanto em I-C, I-T, T'-T, C'-T', C-T, maiores em T2 (p=0,001). No G2, a comparação intragrupo apresentou I-C', ICT, C'T'T, CTT' menor em T1 (p=0,003/p=0,001), e no T2, I-C, I-T, T'-T, C'-T', C-T, IC'T' maiores (p=0,003/p=0,0001). Nas medidas angulares e lineares, não houve diferença significativa entre G1 e G2, em T1 e T2. A diferença intergrupos (∆=T2 T1), mostrou que G1 apresentou IC'T' menor do que o G2 (p=0,045). A oclusão dos arcos não apresentou diferença significativa (p=0,7982) entre G1 e G2.

Conclui-se que, a palatoplastia em dois tempos apresentou melhor resultado para crescimento dos arcos em comparação ao tempo único.

Analysis of the transmission of genetic variants from non-cleft parents to cleft children using machine learning methods

Orofacial clefts are congenital malformations resulting from failures in the fusion of craniofacial processes during embryonic development. Cleft lip, with or without palate involvement, manifested in a non-syndromic form (CL±PNS), represents the most common type of these anatomical alterations. The high genetic heterogeneity observed in mixed populations, such as the Brazilian population, is a challenge for genetic association studies based on traditional approaches. Therefore, this study aims to identify a panel of single nucleotide polymorphisms (SNPs) that shows the genetic transmission from parents without CL±PNS to their children with CL±PNS, using machine learning methods. Seventy SNPs previously associated with CL±PNS in the Brazilian population were analyzed across 364 families (affected individuals and their respective parents) using random forest and neural network models to identify reduced subsets of SNPs with greater discriminatory potential. The significance of the selected markers was assessed using permutation tests. The developed models showed an accuracy of 97.7% in discriminating between affected individuals and their unaffected parents. Among the variants analyzed, 7 presented a significance value greater than 10, but only the SNP rs1106514, located in the MSX1 gene, was able to maintain statistical significance after permutation correction. A significant epistatic interaction was identified between the SNPs rs1801133 (MTHFR) and rs2569062 (TCOF1).

The combined approach of family trios and machine learning was effective in showing the transmission from parents without CL±PNS to their children with CL±PNS, proving promising for the study of CL±PNS.

Long-term systemic effects of radiopacifier accumulation from calcium silicate-based materials: in vivo and human renal cell responses

Radiopacifiers present in calcium silicate-based materials are released systemically, however their long-term biological impact remains unclear. This study investigated the long-term release, organ accumulation and responses induced by released radiopacifiers and explored their translational relevance to human renal response in RPTEC/TERT1 renal cells. Four materials containing different radiopacifiers - bismuth (Bi), tantalum (Ta), tungsten (W) and zirconium (Zr), an experimental material (with gadolinium oxide), and a radiopacifier-free formulation were characterized by XRD and implanted in both subcutaneous and tibial bone of Wistar rats for 30 and 180 days. Elemental migration and accumulation were assessed by X-ray fluorescence, autometallography (AMG) and mass spectrometry (ICP-MS) in kidney, liver and brain, while histopathology assessed biological responses following exposure. Renal immunohistochemistry assessed metallothionein (MT2A) immunostaining. In vitro, RPTEC/TERT1 were exposed to material extracts and MT2A and Kidney Injury Molecule-1 (KIM-1) expression were evaluated by qRT-PCR. Corroborated by AMG and ICP-MS, Bi and W significantly accumulated in renal and hepatic tissues after 180 days. Bi induced pronounced histopathological alterations and, along with W, was associated with increased renal MT2A expression. In cells exposed to extracts containing Bi, MT2A was upregulated, and both Bi and W promoted a progressive overexpression of KIM-1.

Radiopacifiers exhibited long-term accumulation in liver and kidney, inducing biological responses. Increased MT2A expression suggests activated defense against metal, while KIM-1 upregulation indicates the potential to cause kidney injury.

Hyperglycemia Dysregulates Host Response to Dual-Species Biofilms: A Co-Culture Model of Fibroblasts and Macrophages

To determine whether hyperglycemia reprograms host-cell responses to endodontic biofilm products. Periodontal ligament fibroblasts (PdLFs) and THP-1-derived macrophages were co-cultured in a Transwell system and exposed for 48h to normal glucose (5 mM) or high glucose (25.5 mM + AGEs), with or without supernatants from 7-day single- or dual-species biofilms of Enterococcus faecalis and Fusobacterium nucleatum. Cell viability (MTT) and cytokine levels (TGF-β, TNF-α, IL-6, IL-8, IL-10) were quantified. Hyperglycemia significantly modulated host-cell responses in a stimulus-dependent manner. E. faecalis reduced metabolic activity under hyperglycemia (p<.01), whereas F. nucleatum increased it (p<.0001). Dual-species biofilm decreased viability (p<.01), indicating loss of cellular homeostasis. TNF-α increased under hyperglycemia and after bacterial stimulation, with the highest levels in E. faecalis (p<.05). IL-6 remained elevated irrespective of glucose levels, while IL-8 remained unchanged. Hyperglycemia alone upregulated TGF-β and IL-10 (p<.05), suggesting a compensatory regulatory state; however, this response collapsed after bacterial challenge, particularly in the dual-species condition (p<.0001).

Hyperglycemia does not simply intensify inflammation but reprograms host-microbe interactions through enhanced pro-inflammatory signaling, loss of regulatory balance, and stimulus-dependent cytotoxicity. Dual-species biofilm under hyperglycemia drove a dysregulated response, providing mechanistic evidence for the greater severity of apical periodontitis in type 2 diabetes mellitus and suggesting that the TGF-β/IL-10 regulatory axis may represent as a potential therapeutic target.

Posterior crossbite in the mixed dentition is commonly treated with dentoalveolar expansion; however, the effects of different appliances remain unclear. This randomized clinical trial evaluated dentoalveolar changes produced by clear aligners (CA) and removable palatal expanders (RPE) in children with posterior crossbite and Angle Class I malocclusion. Thirty-seven children were randomly assigned to the CA group (n=19; mean age 9.01±1.05 years) or RPE group (n=18; mean age 8.86±1.35 years). Three-dimensional digital models were analyzed using 3D Slicer software at baseline, 6 months, and 12 months. Data were analyzed using Mann-Whitney, Wilcoxon, chi-square with Yates correction, analysis of variance, and Bland-Altman tests (α=5%). Both groups showed significant transverse increases, buccal tipping of first molars, and increased arch perimeter after 12 months (p<0.05). The CA group demonstrated greater expansion in permanent first molars at 6 months, whereas the RPE group showed greater cervical expansion in deciduous canines at 6 and 12 months (p<0.05).

Both appliances were effective for dentoalveolar expansion in the mixed dentition, with comparable overall efficiency and specific biomechanical differences that should be considered during treatment planning.

Development of β-AgVO3 mucoadhesive hydrogel for control of oral microbial and viral infections: in vitro, in silico, and ex vivo approaches

This study aimed to develop a mucoadhesive hydrogel containing nanostructured silver vanadate decorated with silver nanoparticles (β-AgVO3) and to evaluate its physicochemical, antimicrobial, and antiviral properties using in vitro, in silico, and ex vivo approaches. β-AgVO3 was synthesized and characterized by transmission electron microscopy (TEM), scanning electron microscopy (SEM), energy-dispersive X-ray spectroscopy (EDS), and X-ray diffraction (XRD). Three hydrogels were formulated: without antimicrobial agent (G0), 0.12% β-AgVO3 (G1), and 0.12% chlorhexidine (G2). They were evaluated for pH, viscosity, spreadability, syringeability, and ex vivo mucoadhesion. Antimicrobial activity was tested against S. aureus, P. aeruginosa, C. albicans, E. faecalis, and E. coli by agar diffusion and biofilm assays (metabolic activity and SEM). Antiviral activity against SARS-CoV-2 was assessed by in silico modeling and in vitro plaque reduction assay. Data were analyzed by ANOVA, Tukey, and t-test (α = 0.05). TEM, SEM, EDS, and XRD confirmed the successful synthesis of β-AgVO3. G0 and G1 showed similar pH (p = 0.524). Compared with G0, G1 exhibited lower viscosity (p = 0.023), higher spreadability (p = 0.035), and reduced syringeability force (p = 0.007), with similar mucoadhesion (p = 0.079). G1 showed higher antimicrobial activity compared with G0 and G2 (p < 0.05). In silico analysis showed 2 putative binding sites for β-AgVO3 in the M and E proteins of SARS-CoV-2, and in vitro assay showed 80% virucidal activity compared with G0 (p < 0.001).

The β-AgVO3 hydrogel exhibited suitable physicochemical properties and broad antimicrobial and antiviral activity, suggesting potential for oral microbial and viral infection control.

Machine learning-based development and evaluation of a multivariable prediction model for osteoradionecrosis in head and neck cancer

Osteoradionecrosis (ORN) is one of the most severe late adverse effects of head and neck radiotherapy (RT). Predictive models may help identify patients at increased risk and support individualized follow-up. This study aimed to develop and internally validate a machine learning-based multivariable prediction model for ORN in a real-world cohort, following the TRIPOD+AI statement. Clinical variables and mandibular dose-volume histogram parameters were collected from head and neck cancer patients treated with RT. Forward Stepwise Logistic Regression (LR) was used for predictor selection. Six algorithms, LR, Random Forest, Support Vector Machine (SVM), CatBoost, XGBoost, and TabPFN, were evaluated with and without data augmentation, using repeated stratified 5-fold cross-validation. Performance was primarily assessed using sensitivity (recall). Friedman and Nemenyi post hoc tests were applied. Ninety-seven patients were included (ORN: 8.24%). Of the 77 available variables, mandibular volume receiving 60 Gy and tumor location in the oral cavity were selected as predictors. Without augmentation, LR achieved the highest recall (0.74 [CI 0.65-0.83]), while several algorithms showed poor recall. Data augmentation improved recall across all models, with the highest values for TabPFN, CatBoost, LR and SVM (recall 0.77 [CI 0.68-0.86] for all), with statistically significant gains compared with all non-augmented models except LR and CatBoost.

ML demonstrated potential for ORN prediction, with gains in performance following imbalance-handling strategies. LR remained a robust and consistent performer across approaches. External validation in larger and more diverse cohorts is required before clinical implementation.

Notch-epigenetic programs shape therapeutic response in adenoid cystic carcinoma patient-derived xenografts

Adenoid cystic carcinoma (ACC) is a salivary gland malignancy characterized by late metastasis, epigenetic alterations, and limited response to platinum-based chemotherapy, the standard treatment for advanced disease. We aimed to define the genomic programs underlying therapeutic response to epigenetic and platinum-based therapy in ACC using patient-derived xenograft (PDX) models. Three PDX models (ACCX5M1, ACCX6, ACCX9) were treated with vehicle, entinostat, cisplatin, or their combination (N = 36) for three weeks. Whole-exome sequencing was performed on treated tumors, followed by transcriptomic validation using public ACC and ACC-PDX RNA-seq datasets. Associations with clinical and phenotypic variables were evaluated using Wilcoxon tests, correlation analyses, and Cox regression (p < 0.05). PDX models displayed molecular heterogeneity and distinct therapeutic responses. ACCX9 was highly sensitive and exhibited a NOTCH-high/epigenetic-low profile, whereas ACCX6 was resistant and characterized by a NOTCH-low/epigenetic-high state. ACCX5M1 showed an intermediate response and enrichment of adhesion-related programs. Higher baseline NOTCH activity was associated with improved treatment response. Entinostat-based therapies were linked to the loss of variants in oncogenic signaling and DNA repair genes (GNAQ, UIMC1, DUSP5) in responsive models. In patient validation cohorts, NOTCH-high/epigenetic-low signatures were associated with more aggressive tumors and worse clinical outcomes.

These findings support ACC as a disease driven by distinct genomic profiles that shape therapeutic response. Entinostat-based therapy emerges as a promising strategy for ACC with a NOTCH-high/epigenetic-low profile.

Multimodal Analysis of Extracellular Matrix in Oral Leukoplakia Using Nonlinear Optical Microscopy and Raman Spectroscopy

The aim of this study was to compare the extracellular matrix (ECM) of normal tissues (NT) and oral leukoplakia (OL) using a multimodal approach combining non-linear optical microscopy and Raman spectroscopy (RS), and to explore the potential of these techniques as auxiliary diagnostic tools. Samples were subjected to second harmonic generation (SHG) and two-photon excited fluorescence (TPEF) to evaluate collagen organization and elastin fluorescence. Quantitative analysis of collagen intensity and fiber parameters was performed using CT-FIRE and CurveAlign (MATLAB), while elastin was graded semi-quantitatively. RS was conducted with spectra acquired in backscattering geometry and processed using PortoFlow software. A total of 79 samples were evaluated, including 10 controls with normal mucosa and 69 with OL. SHG collagen intensity was lower in OL compared with NT, without statistical significance (p=0.058). At the fiber level, parameters including width, length, straightness, angle, orientation, and alignment showed lower median values in OL, also not statistically significant. TPEF elastin autofluorescence was higher in OL (2.75; 0.0-3.0) than in NT (0.0; 0.0-3.0) (p<0.001). RS analysis revealed differences in the low-frequency spectrum, including the nucleotide-related band, which showed higher intensity in OL lesions and a peak shift from 573 to 576 cm⁻¹. PCA (principal component analysis) of RS data clearly separated healthy and OL tissues.

Our findings suggest that multimodal analysis could represent a promising auxiliary approach for the diagnosis, as ECM remodeling appears to provide additional structural and molecular information that may contribute to earlier and more accurate clinical assessment.

hnRNP-A2 targeting by P. gingivalis PPAD links citrullination to therapy in rheumatoid arthritis

This study evaluated the role of Porphyromonas gingivalis peptidylarginine deiminase (PPAD) at the interface between periodontal disease and rheumatoid arthritis (RA), investigating its reactivity with serum antibodies and its capacity to citrullinate human proteins. Serum samples from patients were analyzed against a citrullinated peptide derived from the enzyme PPAD. Concurrently, recombinant PPAD was expressed and purified for enzymatic characterization. In vitro fluorescent assays were performed to compare catalytic activity across multiple substrates, under varying experimental conditions. The influence of environmental factors, such as the presence of imidazole as a chemical chaperone, was specifically tested to assess protein folding and activity. Qualitative analysis revealed significant functional selectivity, as the enzyme exhibited preferential catalytic activity toward hnRNP-A2. While imidazole notably enhanced wild-type activity, the mutant variant remained unresponsive. This suggests that structural constraints within the mutant preclude the activating effects of imidazole.

The identification of hnRNP-A2 as a key substrate suggests that sequence recognition and environmental triggers directly modulate the generation of neoantigens. These findings indicate that PPAD contributes to the activation of autoimmune responses by generating specific neoantigens, reinforcing the biochemical connection between periodontal inflammation and systemic autoimmunity. The selectivity for hnRNP-A2 highlights relevant immune pathways linked to macrophages and lymphocytes, positioning PPAD as a promising target for biomarker discovery and novel therapeutic strategies in RA.
